Helping the Most Vulnerable: An Inside Look Into Enhanced Care Management
What is ECM?
We don’t just connect them to resources, we make sure people actually receive them.
What are the benefits?
In many cases seniors live alone or are caring for a partner who also has health issues. As a result, their lives are very complex. ECM gives them a partner that helps them find and get resources suited to their needs with hands-on support. Our goal is to keep them as healthy as we can in their own home, as independent in the community as possible, and to minimize preventable healthcare use by connecting them with other, more cost-effective services. Those enrolled can improve the quality and longevity of their life in a healthy way while also reducing the financial burden for them and providers, benefiting the healthcare system as a whole.
How is it making an impact?
ECM is achieving amazing results for some of the most vulnerable members of our community. We recently helped Salvador*, a 78-year-old man living alone with dementia, avoid eviction, reinstate his housing voucher, and access critical care. An ECM case manager coordinated with his doctor, nurse, IHSS, and landlord to improve his living conditions, safety, and health routine, ensuring he could remain safely housed and supported. This is just one of many success stories.

Attended our educational classes so far this year. Our courses provide valuable tips, insights and training that improve care, create a more educated workforce, and inform our community about vital brain health, dementia and healthy aging topics that is creating a more dementia-friendly community.

- In-person groups in 17 different cities across OC
- Groups in English, Spanish, Korean and Mandarin
- Faith-based groups
- Special support groups for unique needs and other forms of dementia
– Adult Day Health Center –
Healthy Aging Center: Acacia offers daytime assistance to hundreds, promoting healthy aging in an enriching environment—at a fraction of skilled-nursing facility costs—giving participants the care they need and their families a break from caregiving.
