I, the Undersigned, am the parent or guardian of the above-named student and wish to allow my minor child to volunteer services to Alzheimer’s Orange County. I acknowledge and agree that the nature of the volunteer services which are typically performed by Alzheimer’s Orange County volunteers, and which may be performed by my child as an Alzheimer’s Orange County volunteer, may involve (a) physical activity, (b) contact with unidentified and unfamiliar persons, (c) travel to and from various unspecified locations, and (d) other potential risk of injury. Notwithstanding the preceding sentence, I willingly and freely agree to have my child volunteer and hereby assume any and all risk, and agree to release Alzheimer’s Orange County for all liability for such risk, including without limitation risk of any accident, injury, illness or death to person or property which my child may sustain, even if caused by the negligent or reckless conduct of an Alzheimer’s Orange County employee or volunteer, in connection with participation as an Alzheimer’s Orange County volunteer or in any Alzheimer’s Orange County related project or activity.

I understand that my child will not receive any monetary compensation for the services contributed or be guaranteed any future position at Alzheimer’s Orange County. The Undersigned hereby grants to Alzheimer’s Orange County the unqualified and perpetual right to use, and consents to the use of, the name and likeness of the above-named child in connection with Alzheimer’s Orange County photographs, worldwide and in any and all media, including, by way of illustration, but not limitation, the display of still photographs, the inclusion on the World Wide Web and the preparation and dissemination of any advertising and promotional materials used to promote Alzheimer’s Orange County. The Undersigned gives permission for the above-named child to be given emergency medical treatment and/or transportation if necessary in the event of accident, injury or sudden illness while said minor is engaged in volunteer service to Alzheimer’s Orange County.

The Undersigned agrees to accept all financial responsibility for any such medical treatment and transport. The Undersigned further acknowledges the following: I have read this release (or have had it read to me) and fully understand each and every one of the provisions in this release and I hereby voluntarily give my express consent and agree to abide by the terms of this release.

AlzOC Waiver
By signing below, I acknowledge that I understand its intent, and I for myself, my heirs,
executors, administrators and representatives, do hereby agree and will absolve and
hold harmless Alzheimer’s Orange County, the sponsors, cooperating organizations and
any other parties connected with this event in any way together with their respective
successors and assigns, singly and collectively, from and against any blame and liability
for any injury, harm, loss, inconvenience or any other damage of any kind whatsoever,
which may arise out of the negligence or carelessness on the part of any person named
in this waiver.
I hereby consent to and permit emergency treatment in the event of injury or illness
while participating in WALK4ALZ 2026. I also hereby give permission to Alzheimer’s
Orange County to use my name or image by way of a photograph, video or audio format
taken of me during the event in any promotional materials, publications or any other
electronic media in addition to disclosing my name on Alzheimer’s Orange County’s
website, which includes online pledging.
Alzheimer’s Orange County reserves the right to refuse or dismiss anyone that may
cause any disturbance or hindrance in any manner that could jeopardize the safety of
oneself or others.

Angels Waiver
I ACKNOWLEDGE THAT THERE EXISTS A RISK OF SERIOUS INJURY (KNOWN OR
UNKNOWN) TO ME, WHICH MAY INCLUDE PERMANENT DISABILITY OR
PARALYSIS, ILLNESS, OR DEATH, IN CONNECTION WITH MY PARTICIPATION
AND I ASSUME FULL RESPONSIBILITY FOR, AND ALL RISK OF, any bodily injury,
death, or property damage that I may suffer for any reason due to my Participation or
from being on Stadium premises or from the condition of any property, facilities, or
equipment used thereon. I acknowledge that I am in good health and physically capable
of Participation.

I agree to immediately advise an Angels Baseball employee of any unsafe condition that
I observe and will refuse to participate until all unsafe conditions observed by me have
been remedied.

I RELEASE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND HOLD
HARMLESS the City of Anaheim; Angels Baseball LP; Moreno Companies, Inc.;
Moreno Baseball LP; Angels Baseball Foundation, Inc.; LAA1 LLC dba KLAA AM830;
Legends Sports, LLC; MLB Entities; and/or each of their respective parent, related,
affiliate and subsidiary companies (altogether, “Company Releasees”) and each
Company Releasees’ respective officers, directors, employees, volunteers, or agents
(collectively, the “Releasees”) of and from any and all claims, causes of action,
damages, liabilities or expenses (collectively, “Claims”), known or unknown, existing
now or in the future, that I, my spouse, heirs, assignees, next of kin and/or legally
appointed or designated representatives may now or hereafter have against the
Releasees, arising in any way out of my Participation or otherwise has arisen or may
arise against the Releasees, and regardless of any fault or negligence on the part of the
Releasees, except to the extent that such Claims are determined by a court of
competent jurisdiction to have been caused by the Releasees’ gross negligence or
willful misconduct. “MLB Entities” means MLB Advanced Media, L.P.; the Office of the
Commissioner of Baseball; its Bureaus, Committees, Subcommittees and Councils;
Major League Baseball Properties, Inc. (doing business in its own name and as Major
League Baseball Productions); The MLB Network, LLC; Tickets.com, Inc.; the Major
League Baseball Clubs and each of their subsidiaries or affiliated entities; any entity
which, now or in the future, controls, is controlled by, or is under common control with
the Major League Baseball Clubs or the Office of the Commissioner of Baseball; and
the directors, officers and employees of the above entities.
