Adult Day Health Care Services

Health and social support for vulnerable adults and adults with dementia or other disabilities enabling participants to remain living at home instead of a nursing facility. The program helps the whole person with a range of daytime care services that support participants mentally and physically. It offers a safe and friendly environment for social interaction with supervised activities and nutritious meals. At the same time, our skilled staff act as health care liaisons, providing a multi-disciplinary assessment, individualized plan of care, nursing care and person-centered activities. It stresses partnership with the participant, family and/or caregiver, primary physician and community
